Practice only — no tournament or exhibition games. Red shirt designation.

Please read carefully before signing:

I, the undersigned parent/guardian, hereby give permission for my child to participate in ACE Hoops basketball programs and activities, including practices, exhibition games, and the Kelowna tournament (May 8–10, 2026). I understand that participation in basketball involves inherent risks including, but not limited to, physical injury.

I acknowledge that:

My child is physically fit and capable of participating in basketball activities
I am responsible for ensuring my child has appropriate medical insurance coverage
ACE Hoops, its coaches, volunteers, and staff are not liable for injuries sustained during participation
Photos and videos of my child may be used for ACE Hoops promotional purposes
I have provided accurate medical information including BC Medical Services Plan number
I will ensure my child attends practices and events as committed
Fees are non-refundable once the season begins
