I, the undersigned parent/guardian, hereby give permission for my child to participate in ACE Hoops basketball programs and activities, including practices, exhibition games, and the Kelowna tournament (May 8–10, 2026). I understand that participation in basketball involves inherent risks including, but not limited to, physical injury.
I acknowledge that:
- My child is physically fit and capable of participating in basketball activities
- I am responsible for ensuring my child has appropriate medical insurance coverage
- ACE Hoops, its coaches, volunteers, and staff are not liable for injuries sustained during participation
- Photos and videos of my child may be used for ACE Hoops promotional purposes
- I have provided accurate medical information including BC Medical Services Plan number
- I will ensure my child attends practices and events as committed
- Fees are non-refundable once the season begins
