Schedule: Practices on Mondays, Games on Wednesdays

Time: 5:00 PM - 9:00 PM

Location: Beairsto School

Who: Grade 6-8 Boys & Grade 7-10 Girls

Cost: $400 / $450 + GST (5%) per player

Early Bird: $400 + GST (before Dec 1) | Regular: $450 + GST (after Dec 1)

Program fee: $400 (early bird) / $450 (regular), plus 5% GST. Pay by e-transfer to pay@acehoops.net — please include your child's name in the e-transfer message. Rental jersey requires a $75 refundable deposit.

Waiver & Release of Liability

Please read carefully:

I, the undersigned parent/guardian, hereby give permission for my child to participate in ACE Hoops basketball programs and activities. I understand that participation in basketball activities involves inherent risks including, but not limited to, physical injury.

I acknowledge that:

My child is physically fit and capable of participating in basketball activities
I am responsible for ensuring my child has appropriate medical insurance
ACE Hoops staff will make reasonable efforts to ensure participant safety
I release and hold harmless ACE Hoops, its coaches, staff, volunteers, and facility owners from any and all liability for injuries or damages sustained during program participation

I grant permission for ACE Hoops to:

Seek emergency medical treatment for my child if necessary
Use photographs or videos of my child for promotional purposes
