ACE Hoops U18 Girls Tryout Registration

Please read carefully:

I, the undersigned parent/guardian, hereby give permission for my child to participate in ACE Hoops basketball programs and activities. I understand that participation in basketball activities involves inherent risks including, but not limited to, physical injury.

I acknowledge that:

• My child is physically fit and capable of participating in basketball activities
• I am responsible for ensuring my child has appropriate medical insurance
• ACE Hoops staff will make reasonable efforts to ensure participant safety
• I release and hold harmless ACE Hoops, its coaches, staff, volunteers, and facility owners from any and all liability for injuries or damages sustained during program participation

I grant permission for ACE Hoops to:

• Seek emergency medical treatment for my child if necessary
• Use photographs or videos of my child for promotional purposes

I have read and understand this waiver and agree to its terms.
